Service Description
Sleep disturbances and/or fatigue are most often the result of many factors. This appointment is for a video call 30-minute sleep assessment. A questionnaire will be completed to determine what behavioral, lifestyle, and/or medical sleep challenges may be present. This appointment does not include a sleep plan or notes. Please book a sleep consultation for a comprehensive evaluation. *Any gift certificates issued at events expire 3 months from the date of the event.
CANCELLATION POLICY
48 Hour cancellation is necessary or a service fee will be assessed. Please call 716.235.1505 or email hello@startwithsleep.com to reschedule your appointment. Appointments may be rescheduled 1 time only. No refunds will be issued for missed/cancelled appointments.
